چکیده

n the early 1980s, Jatene developed the arterial switch peration (ASO), which soon became the preferred surgery or d-loop transposition of the great arteries (TGA). This peration includes translocation of the pulmonary artery and orta above the sinuses, often with a Lecompte maneuver bringing the pulmonary artery anterior to the aorta) and oronary reimplantation into the neo-aortic sinuses. Alhough the complications inherent in the previous atrial witch procedures were avoided, and the left ventricle could emain the systemic ventricle, other post-operative compliations following ASO have been encountered. With inreased surgical experience, the initial problems seen with oronary transfer are now rare. Unusual coronary patterns, nce considered to be a risk factor for survival, have been ound more recently not to affect outcome (1). Supravalvar ulmonary stenosis at the reanastomosis site or branch ulmonary stenosis due to tension has been observed. At hortand longer-term follow-up, neo-aortic root dilation nd neo-aortic regurgitation (AR) are relatively common, nfrequently leading to reoperation.

Mid-term morbidity and mortality of patients after arterial switch operation in infancy for transposition of the great arteries.

INTRODUCTION AND OBJECTIVES The arterial switch operation is currently the preferred surgical approach for complete transposition of the great arteries. We sought to determine the mid-term results of this intervention. METHODS A single-institution retrospective review of clinical records of all consecutive patients who underwent the arterial switch surgery between 1985 and 2010. RESULTS Ove...

Valve-Sparing Reimplantation Technique for Treatment of Neoaortic Root Dilatation Late after the Arterial Switch Operation: Raising the Bar

Neoaortic root dilatation can develop during long-term follow-up after an arterial switch operation (ASO). Although few patients require surgical reintervention, significant valve regurgitation is still an important cause of late morbidity. We report on a 15-year-old boy with significant dilatation of the neoaortic root that was treated with the valve-sparing reimplantation technique. There is ...

Long-term predictors of aortic root dilation and aortic regurgitation after arterial switch operation.

BACKGROUND Neo-aortic root dilation (ARD) and neo-aortic regurgitation (AR) may be progressive after arterial switch operation (ASO) for d-loop transposition of the great arteries (dTGA). We sought to identify predictors of ARD and AR after ASO. METHODS AND RESULTS 335 patients were identified who underwent ASO for dTGA with intact ventricular septum or ventricular septal defect (VSD), includ...

Aortic valve regurgitation after arterial switch operation for transposition of the great arteries: incidence, risk factors, and outcome.

OBJECTIVES The aims of this study were to assess the prevalence and incidence of aortic valve regurgitation (AR) after arterial switch operation (ASO), its outcome, and the risk factors. BACKGROUND After an ASO, the long-term fate of the aortic valve is a concern as follow-up lengthens. METHODS Operative and follow-up data on 1,156 hospital survivors after ASOs between 1982 and December 200...

The importance of neo-aortic root geometry in the arterial switch operation with the trap-door technique in the subsequent development of aortic valve regurgitation.

OBJECTIVE Regarding neo-aortic valve regurgitation (neo-AR) after the arterial switch operation (ASO), the 'trap-door' technique was supposed to be a risk factor due to a distortion of the sinotubular junction (STJ) geometry. Here we report our results of the 'trap-door' technique with a special emphasis on root geometry including the ratio of STJ to annulus. METHODS From August 1991 to March...
